What do I bring to my first appointment?

Bring your smile...and these to your first appointment:
Complete the patient questionnaire to the best of your ability prior to your visit and then we will work with you to review it.
Details of your dental insurance coverage for you and your spouse / partner.

What is your Appointment Policy?

At Smilez Dentistry we are dedicated to providing our patients with the best dentistry and services available. For this reason, we reserve your appointment exclusively for you.

An appointment reminder will be sent to you by text, email or voice call two weeks as well as four days prior to your appointment. If for any reason you cannot keep your appointment, we require two business days advance notice in order to provide all our patients, including yourself, with timely and quality care.

What is your Payment Policy?

The cost of dental treatment varies depending on individual needs and treatment plans. The costs and benefits of various treatment options will always be discussed with you before we select a personalized treatment plan. We accept all major credit cards for payment for dental work performed at Smilez Dentistry

While your dental insurance is an arrangement between you and your coverage provider, we will be happy to help you understand your coverage as well as with the paperwork. We also will be able to directly submit your claim as we are set up with many insurance companies. Patients, however, are responsible for any remainder amount not covered by their insurance plan. Payment for dental services is due at the time that services are provided.
